We have looked at your recent comments about the way patients book their appointments. As a result the trial which started in April has been adapted as below with effect from 19th July 2010. Please continue to let us know how our system is working - both positively and any problems you experience for our next review in September.:
FOUR WEEK: A percentage of appointments will be made available up to four weeks in advance for patients to book.
48 HOURS: We will be offering a high percentage of GP appointments for patients wanting to book within two working days.
PLEASE REMEMBER WHICHEVER CLINIC ONE PROBLEM PER APPOINTMENT
SAME DAY: Same day URGENT GP and nurse appointments will run in conjunction
with an option for a telephone call if more appropriate.
These are for urgent problems only.
Appointments can be made during normal working hours by calling into the surgery or by telephone. Doctors appointments can be booked up to one week in advance. Please note that each appointment is for one person only. If you have more that one problem to discuss then please ask the receptionist for a double appointment.
On line appointments can be made from the EMIS On Line web site: www.emisaccess.co.uk. Before you can use this system you will need to have created an account - see one of the receptionists for further details.
We offer a triage system for ‘on the day’ problems. The receptionist will take your details and you will be put on the triage list and a clinician will then call you back to discuss your symptoms with you. If they cannot resolve your query over the phone, they can arrange for item(s) to be collected (e.g. prescriptions) or arrange a same day appointment. Please not that they will not be able to see you for non-urgent problems.
Please be considerate and only use this number if you need to contact a doctor urgently. Alternatively dial 999 or contact NHS Direct on 0845 46 47.
A home visit can be arranged if an illness prevents a patient getting to the surgery. There are no set times for these visits - they are normally made in the morning or lunch time. They may be graded as to priority or severity and therefore the receptionist will need to take some details of your symptoms; a doctor may telephone prior to making the visit so please be ready to receive such a call.
At the time of investigation you will be advised the likely date we will receive your results. For reasons of confidentiality unless the patient is under age or we have their written consent, we can only discuss any result with the patient themselves. Please telephone for these results between:
| Monday | 1:00pm - 2:45pm |
| Tuesday | 2:00pm - 2:45 pm |
| Wednesday | 1:00pm - 2:45pm |
| Thursday | 1:00pm - 2:45pm |
| Friday | 1:00pm - 2:45pm |
Please also note this service will not be available on Target afternoons - see the dates for Target afternoons in the STAFF TRAINING AND TARGET section of the Training Practice Page.
The medications query line is available between 10.30am and 12.30pm Monday - Friday (except Bank Holidays). A dedicated member of the reception staff will be able to assist you with general queries about your medication.
All requests must be in writing and we require two working days to process repeat prescription requests. Please allow plenty of time when putting in your request so that you do not run out of medication.

| Monday | Wednesday Afternoon |
| Tuesday | Thursday Afternoon |
| Wednesday | Friday Afternoon |
| Thursday | Monday Afternoon |
| Friday | Tuesday Afternoon |
There are several ways to request your repeat prescription:
If for any reason you have to cancel an appointment please let us know. This is important as it allows another patient to make use of a very valuable appointment slot. You can do this either by:
Coming into the surgery in person to cancel and/or rebook. Telephone during surgery hours and speaking to a Receptionist Leave a message on our answerphone service which is available throughout the day or night.
For private correspondence and medicals (PMA) the fees have been changed recently. If you require anything to be done please ask the Receptionist how much the cost will be so that you are aware of this before the doctor completes any correspondence for you.
When you complete any claim form, solicitor enquiry, insurance proposal or similar, the paperwork provides an option to view the report before it is issued, a right under the Medical Records Act. When you ask for this option we will contact you upon completion of the paperwork to advise you it is ready for viewing. If you have not viewed the paperwork within the statutory 45 days we will forward it to the enquirer without further delay.
